Urgent Notice: Physicals

In recent years the state of NJ has made significant changes to the physical requirement to participate in athletics and intramural activities. Since all students at SBP are encouraged to participate in athletics and/or intramurals we have extended these requirements to all students.

Here is what you need to know and do:

Physical Forms:

The state now issues a four-page form to be used. The first two pages are a health history for the parent to complete prior to seeing the physician. This history must accompany the student when he actually sees the physician. The second two pages are the actual physical examination form to be completed by the doctor. The clearance form must be properly signed and stamped for the physical to be acceptable All of these pages must be submitted to the school once the doctor has completed the physical.


Doctors: It is strongly recommended that students have their physicals completed by their own primary care physician. Physicians performing school physicals are now required to complete a Student Athlete Cardiac Assessment Professional Development module prior to performing physicals. This is an online module. The physician will receive a certificate upon completion of the module and should provide a copy of that certificate along with the completed physical form.

Information sheets: In addition, should also receive the Sudden Cardiac Death in Young Athletes pamphlet, the Sports Related Concussion and Head Injury Fact Sheet, and the Sports Related Eye Injuries Fact Sheet. There is a signoff form for the first two of these confirming that you have received them.

Consent Forms: Finally, there are 3 forms of consent that must be completed. The first two are medically related, the third is consent to participate.

Timing: Physicals are only valid for 1year. Because of the school calendar, it is most helpful if the Physicals are completed in June or July. We realize the challenge this may create with insurance if his previous physical was after July. For those that may have these conflicts, we are suggesting that you take this one year to pay for a physical out of pocket (without reporting to insurance) and not have another physical on insurance until next July. By doing this he will be able to be on annual schedule for physicals in July. (To assist with this, we have arranged to offer reduced price physicals to students at Newark Beth Israel Hospital. These will be completed on the week before school starts in July).
